Questions: So I have new master cyl; new outlet/eye bolt; new brass 'Y' and two new crush washers. Still leaking. The two washers appear to be different inside diameters but the washer with the smaller insider diameter wouldn't fit onto the shoulder of the eye bolt. Should they be the same? Did I mix up one of these crush washers with the new washers for the flex brake lines?

I would double check for any wear on your outlet bolt. You may just need to tighten it up more as well. Also check your brake light switch.
